Facial Plastic Surgery Questions and Answers: Part 04

Question: What can be done for my nose?
Answer: A closed rhinoplasty approach can accomplish reduction of the bulbous nasal tip, shaving down the dorsal hump, reducing the hanging columella, and straightening the caudal septum. All incisions are placed on the inside of the nose. The nose can be slightly de-projected as well as straightening out the asymmetric bridge line.

Question: What would be the best approach for me to take for my crooked nose/deviated septum?
Answer: A closed rhinoplasty approach can accomplish shaving down the dorsal hump, straightening the nasal bones, adjusting the nasal tip cartilages with all incisions placed on the inside of the nose. A septoplasty can be performed on the inside of the nose if there is a deviated septum present to improve airflow dynamics. Both procedures can be performed together under one anesthesia with one recovery period. To prevent the tip of the nose from drooping dynamically when you smile, it is important to release the depressor septi ligament.

Question: How would you enhance my profile?
Answer: The limited Photograph demonstrates an overly projecting nose and an under projecting chin. A closed rhinoplasty approach can accomplish shaving down the dorsal hump, decreasing the overall projection of the nose, refining the nasal tip and reducing the hanging columella with all of the incisions placed on the inside of the nose. No external incisions are required. Consider placement of a chin implant to augment the chin forward for better facial balance and proportions, especially with respect to the over- projection of the nose from the side profile. Both procedures are frequently performed together in this type of scenario.

Question: How many days until I can open my eyes and resume normal activities?
Answer: Patients are able to open their eyes and get around the next day after eyelid surgery. When performing eyelid surgery together with a rhinoplasty, there is additional swelling and bruising underneath the eyes as well. In our practice, these procedures can be performed together under general anesthesia, however you will need a caretaker to be with you for the first 24 hours after the procedure. We also ask patients to refrain from any exercise for 2 weeks after both procedures.

Question: Can I reduce my dorsal hump without altering my tip?
Answer: A frontal photo of your nasal tip would be required to answer your question. If your nasal tip cartilages are narrow enough, they do not need to be altered during the hump reduction rhinoplasty.

Question: Length of time for an extended deep plane facelift?
Answer: It’s best not to measure the results of your facelift procedure by the amount of time it takes a surgeon to perform the procedure. Study the before and after photographs of your perspective surgeon’s facelift photo gallery. Make sure the hair is pulled back behind the ears so that you can see the placement of the incision. In addition, make sure the facelift photo gallery has lots of before and after examples for you to view demonstrating great results. In our practice, a lower face and neck lift takes approximately three hours.

Question: Would a lower blepharoplasty minimize the appearance of hereditary dark circles or should I consider other options?
Answer: A trans conjunctival lower blepharoplasty simply removes the fat pockets creating the puffy look on your lower lids. The incision is completely located on the inside of lower lid. The color of the skin will not change.

Question: Deep plane facelift for African American skin – is this type of facelift good for darker skin tones?
Answer: The type of SMAS tightening used during a facelift doesn’t matter with African-American skin. What matters most is how the skin incision is meticulously closed under no tension, which is trying to avoid any hypertrophic scarring or keloid formation. Follow up with your operative surgeon is also important to monitor for any healing issues for several months after the procedure.

Question: Non-invasive options to address heavy eyes and prominent orbital bone?
Answer: Your side profile shows fat deposits located in your neck, so consider liposuction to address removal of those fat deposits. Your eyelids appear normal and nothing need to be done at age 29.

Question: Am I a candidate for rhinoplasty to straighten nose, define tip & thin bridge?
Answer: Yes, a closed rhinoplasty approach can accomplish shaving down the dorsal hump, refinement of the bulbous nasal tip and narrowing the bridge line with all the incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. Digital computer imaging of your nose upon your facial features would also be helpful to understand what can be accomplished with the procedure. Rhinoplasty is the most difficult operation to perform correctly in the entire field of cosmetic surgery, so choose your surgeon very wisely based on extensive experience producing natural results.
