Facial Plastic Surgery Questions and Answers: Part 4

Question: Revision rhinoplasty and mid-face lift at the same time?
Answer: A full set of facial photographs are required to make a determination about how best to proceed, since revision rhinoplasty is one of the most difficult procedures to perform correctly in the entire field of cosmetic surgery. A facelift and a rhinoplasty can be performed together under one anesthesia with one recovery period. In our practice, we do not perform a mid facelift, since the mid-face does not descend vertically with the aging process. When you lose volume in the mid face, we place cheek implants to restore the lost volume.

Question: Revision rhinoplasty. Can my nose be more defined and feminine?
Answer: Much more information is needed, such as a full set of facial photographs from all angles. In addition, it is also important to know how much cartilage is left over on the inside of the nose if cartilage grafting techniques are required. It is also probably best to have a copy of the operative report to find out what procedures were accomplished by your primary rhinoplasty surgeon. Revision rhinoplasty is more difficult than a primary rhinoplasty, therefore in-person examination is going to be required.

Question: My nose grew bigger 5 years after rhinoplasty. Am I a good candidate for a revision?
Answer: Its very difficult to tell from photographs presented, but is probably best to go back to your original surgeon. If there is thickened skin in the nasal tip, Steroid shots may be of some benefit. If the width of the tip is caused by excess cartilage, then a revision rhinoplasty would be needed.

Question: Can someone give me a nose job?
Answer: The photographs presented of yourself demonstrate a wide nose and a recessive chin profile.  Consider a closed rhinoplasty to narrow the bridgeline and nasal tip, overall neck nose smaller.  Consider placement of a small chin implant to augment the chin forward for better facial balance anterior portions from the side profile. Do not expect to get the “Instagram model look”. Digital computer imaging on both your nose and chin can be performed through our office by clicking on the virtual rhinoplasty button on the link below.

Question: Does my nose fit my face?
Answer: The decision to undergo a cosmetic rhinoplasty procedure must be made by the patient, and not someone else. It is a very personal decision. If the dorsal hump and twisted nose bothers you a lot, then undergo the procedure as long as you are in excellent health and your BMI is less than 34. A closed rhinoplasty approach can accomplish shaving down the dorsal hump, straightening the bridge line, and making any minor adjustments to the tip, if needed. All the incisions are placed on the inside of the nose.

Question: How can I fix my hooked nose?
Answer: A closed rhinoplasty procedure can accomplish shaving down the dorsal hump, and narrowing the bridge line just like in the digitally altered photograph. All the incisions are placed on the inside of the nose. No external incisions are required. If your tip droops dynamically when smiling, it is very important to release the depressor septi ligament which prevents the tip from drooping when smiling after the surgery.

Question: Rhinoplasty to project the tip to thin the skin and redefine it – is this a good option?
Answer: From the photographs presented, you have an overly projected nose, and an under projecting chin. A closed rhinoplasty approach (not open rhinoplasty) can accomplish reduction of the projection of your nose, shaving down the hump, and narrowing the nose from the frontal profile. All the incisions are placed on the inside of the nose. No external incisions are required. Placement of a chin implant Will help significantly with facial balance and proportions, by augmenting the chin forward. In our practice we make that incision underneath the chin to place the implant. Both procedures can be done together under one anesthesia with one recovery period. Choose your surgeon wisely based on extensive experience, since rhinoplasty is the most difficult procedure to perform correctly in the entire field of cosmetic surgery.

Question: Rhinoplasty candidate for more feminine nose that’s not so long?
Answer: A closed rhinoplasty approach can accomplish making the entire nose smaller, and shorter with all incisions incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. Choose your surgeon wisely, since rhinoplasty is the most difficult procedure to perform correctly in the entire field of cosmetic surgery.

Question: Can a rhinoplasty cause permanent damage/paralysis to the eye muscles?
Answer: No, the eye muscles are not anywhere close to where a rhinoplasty is being performed anatomically. Choose your surgeon wisely, since rhinoplasty is one of the most difficult procedures to perform correctly in the entire field of cosmetic surgery.

Question: Can I have a rhinoplasty after I had Botox for bunny lines?
Answer: It is certainly acceptable to undergo a rhinoplasty procedure Before or after Botox. Rhinoplasty involves re-shaping the cartilage and bone in the nose. Botox only temporarily paralyzes the muscle on the bridge of the nose and will have no bearing on the structural bone and cartilage in the nose itself.
